The story as I heard it: Someone broke the transit so they eyeballed the last few towers. They were far enough off that it repeated derailed, even with the most extreme settings of the shiv wheels. The reason this won't be the right answer is that it was never repaired!

I believe it! As someone who has personally delt with the fickleness of Hall T-Bars, if you look at them funny they are prone to derailing. And that is with them in alignment!
